what i don't understand is why he was kept being played every match until the international break in the early part of the season, when everyone knw it would make it a chronic injury and could affect his whole career. why?

he have been playing with the same injury since april 2010. inlcuding world cup, yes. and it's still bothering him but he never complains. last 3 seasons he have played like 60-65 games per season which is pretty crazy. but now that he has won every single trophy in football and he has that nasty injury, maybe he should retire from NT and concentrate fully in barca. paul scoles retired too NT but he still plays with manu.
